10-WEEK VIRTUAL PROGRAM

Join us for a unique program that will help you:

• Plan and create your housing vision for the future

• Explore various individualized housing & support models

• Learn about ways you can maximize financial resources

• Discover strategies to reduce support costs

• Identify goals and tasks to begin implementing your plan

SERIES INCLUDES:

Group learning sessions and individual support of a Planning Facilitator

Dedicated time with your Planning Facilitator between learning sessions to support your planning process
___________

Individual consultation with Housing Specialist Kelly Casey to optimize your plan
___________

Preparation of your individualized housing plan complete with a budget
___________

Meet with and learn from other families interested in individualized housing
___________

Cost for the series is $1600 (HST included) and is an eligible expense for Passport Funding.

ANNA BRUNO, MANAGER OF PLANNING & FACILITATION, INDEPENDENT FACILITATOR 

Anna has worked in a variety of capacities supporting people living with disability to envision and live out their best lives for over 20 years. In addition to her work at P4P she is a community host and member of the Planning & Facilitation Collaborative and is an educator, teaching Person-Directed Planning to students in the Developmental Service Worker Program at Centennial College. Anna belongs to the circles of support of a few close friends, and this offers an opportunity to stay grounded in the work as well as build a deep knowledge of how circles function and how the relationships within them support people towards achieving a good life, rooted in their community.

ANDREA JOHNSTON, INDEPENDENT FACILITATOR + PLANNER

Andrea has been connected to the developmental service sector for more than 30 years promoting community inclusion and identifies as a changemaker. She has held many roles within the developmental disability sector. As the creator and coordinator of the All Abilities Dance Troupe, Andrea is passionate about spreading the message of possibilities through the groups’ community performances. By creating space for people of all ages and abilities, Andrea invites us all to safely explore the endless possibilities that planning can achieve as an Independent Facilitator. Andrea is also a registered Yoga teacher and personal trainer with certificates in various natural healing modalities.

STACEY GREEN, INDEPENDENT FACILITATOR + PLANNER

Stacey is enjoying retirement after working for a decade in municipal recreation providing inclusive community-based programs and services for individuals of all ages and abilities—followed by 20 years teaching Recreation & Leisure Studies and Recreation Therapy at Niagara College. Stacey volunteered to champion the role of recreation in children’s healthy development on the board of directors for Pathstone Mental Health for 6 years.  She began working with Partners for Planning as a Regional Partnership Coordinator in 2021 and now guides individuals and their families through the Crafting Your Individualized Housing Plan Workshop series.

CARRIE LEO, INDEPENDENT FACILITATOR + PLANNER

With over 15 years of experience as a faculty member at Mohawk College, Carrie is skilled in teaching, advocacy, and community development. She has held many roles within the developmental disability sector and is very proud to have, in 2008, promoted Zambia's inclusion in the UN Convention on the Rights of Persons with Disabilities. Currently, she is also a Research Assistant and Job Coach with CanChild’s Autistic Youth Employment-Ready Program, supporting neurodiverse youth in achieving employment goals. As the founder of Care Unique Planning, Carrie has provided independent facilitation and planning services for over five years, supporting individuals through various life stages, including pathways and circle development.

KIRA ORENDORFF, INDEPENDENT FACILITATOR + PLANNER

Kira’s journey into independent facilitation began while supporting a family member with a developmental disability. Inspired by the positive impact of personalized support, she helps families build lives rooted in community, choice, and belonging. With a Master’s in Applied Positive Psychology and over 15 years of experience in relationship-building, strategic planning, and project management, Kira combines practical expertise with a strengths-based, heart-centred approach. She has a facilitation practice in Toronto, partnering with families to create clear, sustainable plans tailored to each person’s unique needs and goals. Kira also works as Project Manager for Microboards Ontario helping people incorporate Microboards to create a supportive, person-directed, not-for-profit corporation. Passionate about inclusion and collaboration, Kira is dedicated to building lasting plans that bring confidence and peace of mind.

RIMA ALSALAH, INDEPENDENT FACILITATOR + PLANNER

Rima is an educator, therapist, and independent facilitator with over 20 years of experience in disability justice and family support. Her work is grounded in Applied Behavioural Analysis, as well as Special Education and aims to challenge the social, legal, and institutional barriers that restrict the independence of people with intellectual disabilities, particularly women and girls. Having worked throughout the Greater Toronto Area, Rima has helped families achieve their vision for their loved ones. As a parent of a son with an intellectual disability, she understands that systemic issues, rather than individuals, often create most barriers. She assists families in developing inclusive models, managing transitions, and advocating for systemic reform to ensure their plans remain sustainable.
